The Accreditation Cycle Repeats Itself

In August 2008, QMP–LS will complete all initial assessment visits in Ontario licensed laboratories. Every laboratory should expect to be reassessed before its current certificate of accreditation expires. The Ontario Laboratory Accreditation (OLA) issued the first reassessment notices in May 2008. 

How much notice do laboratories receive?
Laboratories will receive 120-day advance notice that their reassessment visits must occur.

When can laboratories expect notification?
OLA must issue new certificates before the existing certificates expire. To accomplish this, OLA calculates the reassessment notification date by working backwards from the expiry date on a laboratory’s certificate of accreditation (Figure 1).

To determine the exact date of notification, the following timelines are considered:

  • Four month’s preparation time is allowed prior to the assessment (120 days notice).

  • Three months is allowed following the assessment, for corrective action plans to be submitted.

  • Two months is allowed for review and approval of submitted corrective actions and if necessary, for submission of additional evidence, before certificate issue.

Therefore, laboratories can expect to receive their reassessment notices nine months prior to certificate expiry dates.

Who qualifies for a multi-site assessment?
Laboratories with the same management and quality management system are eligible for a multi-site assessment. If your laboratory is part of a corporate grouping that was not in place during your previous assessment visit, please contact ola@qmpls.org, to let us know you would like to adjust your assessment 


QMP–LS policy on confidentiality, conflict of interest and privacy

QMP–LS Information Management Policy

QMP–LS manages the flow of information within and outside of the organization in a manner that protects its integrity as well as the confidentiality of participating laboratories and the privacy of individuals in accordance with applicable statutes, regulatory and accreditation requirements.

The system includes processes and procedures to:

  • Ensure appropriate receipt, storage and distribution of information

  • Protect security of information including intellectual property, corporate logos and symbols

  • Protect confidentiality of information and privacy of individuals

  • Ensure ethical, impartial and objective use of information

  •  Protect the integrity and ownership of QMP–LS information

Committee Members and Volunteers

It is mandatory for all QMP–LS committee members and volunteers to read, review, understand and comply with the QMP–LS Conflict of Interest Agreement. Each member/volunteer receives this form during orientation. A signed form must be submitted to QMP–LS. If a member/volunteer identifies a conflict of interest, the Potential Conflict of Interest Declaration must be completed, signed and submitted to QMP–LS.

A terms of reference is available for each of QMP–LS’ committees. Each terms of reference includes a comprehensive discussion on the committee’s:

  • mandate,

  • membership,

  • appointment,

  • term of office,*

  • responsibilities,

  • accountability,

  • chair,

  • quorum,

  • meetings,

  • expenses, and

  • record of meeting.

*The Term of Office section of each committee’s terms of reference reinforces QMP–LS’ commitment to safeguarding confidential information:

  • Persons recommended for appointment to the committee shall declare a potential conflict of interest according to
    QMP–LS policy.

  • Persons appointed to the committee shall consider all material disclosed to them in regard to or arising from this appointment as confidential.

Contract Personnel

All contract personnel are also required to comply with QMP–LS’ Information Management Policy by signing the QMP–LS Conflict of Interest Agreement and declaring any conflicts of interest using the Potential Conflict of Interest Declaration form.
