QMP–LS is pleased to announce that we are subcontracting with Gamma Dynacare Medical Laboratories (GDML) for the packaging and distribution of EQA survey material for chemistry, hematology and transfusion medicine. This will occur in concert with the relocation of QMP–LS offices during the last week of October 2011. The first surveys to be distributed from the GDML Brampton location will be the surveys for enzymes (ENZY-1110), cardiac markers (ENZY-1110-CM) and lipids (LIPS-1110). This change in distribution location should not have any noticeable impact on participants.

In addition to the distribution of the initial survey material, GDML will distribute replacement material (for lost material or material damaged in transit) during the survey testing period and retest material (as part of a discordant findings investigation) after the survey has closed. To make the distribution of these samples as efficient as possible, we will establish single distribution dates for each of these occasions. Requests for material should still be made to QMP–LS. Those received prior to the distribution dates will be held and processed in order of the distribution dates.

Replacement Material. The latest date for requesting replacement of lost or damaged material will be included in the survey instructions received with the material and posted in the General EQA\Survey Information folder for the survey.

Retest Material. The latest date for requesting material for retesting as part of a discordant finding investigation will be included in the discordant finding investigation form which will be posted in each laboratory’s correspondence file following issue of the laboratory survey report.
